How SPRAVATO® Works

When depression does not lift after multiple treatment attempts, it can feel discouraging and confusing. You may be doing everything you can, yet your mood, energy, and motivation still do not return in a way that feels steady. Understanding how SPRAVATO® works can help you decide whether an appointment to explore advanced options is the right next step.

At The Nexus Group PC, we provide outpatient psychiatry and medication management with a focus on evidence-based care. If you are researching SPRAVATO® treatment, you are likely looking for a structured, closely monitored option that fits your real life and does not require guesswork. We guide you through eligibility, scheduling, and insurance steps so you can move forward with clarity.

What Makes SPRAVATO® Different From Traditional Antidepressants?

Most antidepressants primarily influence serotonin, norepinephrine, or dopamine pathways. SPRAVATO® works differently because it contains esketamine, which acts on the brain’s glutamate system. Glutamate is involved in how brain circuits communicate, adapt, and rewire over time. For some people with treatment-resistant depression (TRD), targeting this system offers a new path when standard approaches have not provided enough relief.

The key point is not that SPRAVATO® replaces every other approach, but that it expands what is possible. This supervised, in-office treatment plan is designed for adults with depression, and it can be integrated into a broader outpatient strategy.

How SPRAVATO® Is Used For Approved Conditions

SPRAVATO® is FDA-approved for adults with TRD. It is also FDA approved for depressive symptoms in adults with MDD with acute suicidal ideation or behavior when used alongside an oral antidepressant. Eligibility is based on diagnosis, treatment history, and clinical factors we review during your appointment.

In your appointment, we will review what you have tried, how long you tried it, and what outcomes you experienced. From there, we can discuss whether esketamine nasal spray therapy aligns with your needs or whether another option may be a better fit.

What Happens During Your SPRAVATO® Treatment Appointment

SPRAVATO® treatment is administered in-office under direct supervision, followed by an observation period. This structure is not meant to be intimidating. It is meant to keep you supported, monitored, and safe while the treatment is active in your system. You will also need a plan for transportation home after the appointment.

During your visit, our team follows a consistent process, with room for individualized support. Monitoring commonly includes:

  • Reviewing how you are feeling that day
  • Checking blood pressure and other vital signs
  • Confirming your treatment plan and dosing schedule
  • Supporting you during the supervised self-administration
  • Observing you for at least two hours after dosing
  • Making sure you are clinically stable before you leave

After monitoring, we will give you clear guidance for the rest of the day and confirm next steps for your schedule. Many patients appreciate that SPRAVATO® treatment is organized around repeatable appointments, rather than relying on trial-and-error changes at home.
